Our story
How it started
Fall for Community was started in 2025 by Zeina, owner of New Phase Navigation. It began simply: she invited friends to come together during Homelessness Awareness Month, bring whatever they could, and spend an afternoon assembling donations for their community.
The name comes from the season it began in — and from the invitation underneath it all: fall for your community. Notice it. Get attached to it.

Participants also took care packages home to keep in their cars, so they'd have something to offer when they crossed paths with a neighbor who could use one.

Fall for Community was founded by Zeina and is an initiative of New Phase Navigation, a healthcare navigation and case-management company. The work of connecting people to what they need is the same instinct behind this event — it just happens around folding tables, with neighbors.
